  • All you need to know about the spermogram in the ISIDA clinic

    If the couple has problems with conception of a child, a test for infertility usually begins with a man. It is faster, easier and cheaper. Doctor prescribes the main test – a spermogram.

    Spermogram is a laboratory analysis of a male sperm, which is performed to estimate the fertilizing capacity of semen. In addition, the spermogram makes it possible to suggest the existence of chronic, inflammatory, and sometimes even oncological diseases in a reproductive system of a man.

    The first indication for ordering the semen test is the absence of children of a couple for 12 months after total refusing from any methods of contraception. Sometimes spermogram is prescribed to estimate the fertility disorders after a man’s illnesses. For example, in case of prostatitis, problems in the hormonal system functioning, injuries, some infectious diseases. Test of sperm in such cases allows us to identify men’s abnormalities in the reproductive function in the early stages and prescribe the right treatment.

     

    Basic indicators in the spermogram

    1. The volume of ejaculate is one of the most important characteristics of a man’s fertility. The norm is 2 ml or more. A small amount of sperm can become an obstacle for fertilization. If there is a significant reduction in volume, doctor can establish a diagnosis of “oligospermia”. The volume of seminal fluid is important in terms of the number of spermatozoa present in it, capable of fertilizing an oocyte.
    2. Concentration of spermatozoa. Normally there should be about 15 million in 1 ml of seminal fluid; at least 32% of them should have normal mobility.
    3. Speed ​​of liquefaction of ejaculate. Increased viscosity of the sperm makes it possible to make an assumption about disorders in the prostate gland.
    4. Morphology of sperm (the number of spermatozoa with a normal structure). If there are problems with reproductive health in the semen of a man, we may observe some sperm anomalies of the structure – sperm cells without a head, with two tails, without tails. Normally, there should be at least 4% of spermatozoa with a normal structure.

    How to prepare for the sperm test

    The male sperm reacts quite sensitively to various external factors. Therefore, a single sperm donation for analysis does not give a reliable picture. The vast majority of clinics (when examining a man for infertility) suggests doing this twice, with an interval of about 1 month.

    In order for the results to be as reliable as possible, it is necessary to give the semen for test not before than 48 hours after the last sexual contact and not later than a week after it. Two days before the test it is necessary to refuse from drinking, smoking, visiting the sauna and the bath. Man’s fatigue, recently experienced severe stress, the use of certain drugs – all these factors can also affect the results of the spermogram.

  • What you need to know about childbirth after 35 years

    A modern woman is craving for full self-realization, therefore sometimes postponing the birth of a child for the sake of creativity or the desire to make a career. Sometimes the joy of pregnancy comes in a more mature age for social or for health reasons. Nevertheless, ISIDA doctors say – over the past 15 years, the number of women who decide to become a mother after forty has doubled. In this trend, there are many advantages: a mature woman takes a closer look at her health, listens to the recommendations of doctors and seriously prepares for childbirth. Despite the “age” pregnancies in the ISIDA clinic are viewed from a positive perspective, the bearing of a child, when the “woman is over … years old”, has some peculiarities.

  • Treatment of male infertility in ISIDA clinic

    Often in couples who have problems with the conception of a child, it is women who blame themselves for infertility. In fact, in 40% of couples, infertility is associated with a male factor. Male infertility – a reduction of sperm fertilizing capacity. Causes of infertility in men are diverse: abnormalities of testicles, infectious and inflammatory diseases, chromosomal abnormalities, toxic effect of environmental pollution, exposure to high temperatures, trauma.

    Examination of the pair begins with the man, because the diagnosis of male infertility is easier, cheaper and requires fewer medical interventions. To understand if there are problems with spermatozoa, a man gives sperm analysis. Normal indications spermogramma – 20 million spermatozoa in one milliliter of sperm, half of them – actively mobile. 5 million of them should be without pathologies. Only then sperm is considered qualitative. But if the parameters of the spermogram don’t correspond to the norm, on the basis of a complex examination of the pair, the method of assisted reproductive technologies is individually selected.

    ISIDA clinic uses the most modern methods of treatment for male infertility: artificial intrauterine insemination (IUI), IVF, ICSI and TESE methods.

    IUI is prescribed for infertility, both female and male. In this case, spermatozoa are injected directly into the uterine cavity. Fertilization occurs naturally in the body of a woman.

    IVF is also used for male infertility, when there are problems with male fertility (at least 5 million active-mobile spermatozoa) or after three unsuccessful attempts at insemination.

    ICSI and TESE make it possible to achieve fertilization of the ovum and pregnancy, even in the most severe forms of male infertility. ICSI is indicated in cases when the level of pathological spermatozoa is increased in sperm and they have low mobility. Sperm injection is carried out in the embryological laboratory: with the help of micromanipulators, the selected sperm of good quality is introduced into the cytoplasm of a mature egg. And the TESE method is used when there is very little or no spermatozoa in the sperm. With this method of treatment, a testicular biopsy is performed and spermatozoa are released directly from the resulting tissue. Thus, the couple has a chance to conceive their baby even in the most difficult male factor infertility.

    Reproduction specialists of the in vitro fertilization department Kseniya Khazhilenko and Natalia Redko visited the workshop “POSEIDON criteria: from poor ovarian response to the concept of low prognosis”. The master class took place on October 13-14 in Athens (Greece).

    The workshop in Athens was devoted to the work with an extremely complex group of patients participating in ART programs (assisted reproductive technologies) who received a poor reproductive prognosis due to age or genetic characteristics of the ovaries. Unfortunately, there are a lot of such patients not only in groups having problems with fertilization, but also among patients who experience unsuccessful results of specialized treatment.
